iCAN: A Patient Support Program
Who Can Enroll
A patient may be enrolled in the iCAN Program if they are:
● Diagnosed with cancer (e.g., early, advanced, or metastatic breast cancer) and prescribed
TRASTUMED by their attending physician
● Prescribed ANAGREBENE for thrombocythemia
Enrollment is initiated through the attending physician.
Program Benefits
An enrolled patient may be entitled to avail of the following, subject to program requirements and validation:
● FREE 2D Echo for every purchase of
○ 3 vials of TRASTUMED 440 mg, or
○ 6 vials of TRASTUMED 150 mg
● FREE CBC for every purchase of
○ 6 boxes of ANAGREBENE
● Initial FREE 2D Echo prior to start of treatment, as required and requested by an iCAN-enrolled doctor
● FREE 2D Echo every 3 cycles with Protocol Bundling purchase
● FREE medicine per Treatment Protocol, based on program guidelines
All benefits are subject to compliance with documentation and validation requirements.
How to Enroll in iCAN
Step 1: Receive the iCAN Program Kit
Eligible patients will receive the iCAN Program Kit, which includes:
● Dear Patient Letter
● Patient Enrollment Form
● Patient Consent Form
● Patient Care Kit
Step 2: Prepare the Required Documents
The following must be completed and submitted:
● Accomplished Patient Enrollment Form and Patient Consent Form
● Valid government ID
● Doctor’s prescription
● Laboratory request from the attending physician
● Proof of purchase of the prescribed IE Medica product
Additional requirements, depending on the benefit to be availed:
● Laboratory request for initial FREE 2D Echo, or
● Purchase of:
○ 6 vials TRASTUMED 150 mg or 3 vials TRASTUMED 440 mg (for FREE 2D Echo), or
○ 6 boxes ANAGREBENE (for FREE CBC)
Step 3: Submit for Validation
Completed requirements must be submitted to the IE Medica Patient Relations Office for validation and processing of the Letter of Endorsement (LOE).

Step 4: Avail of Diagnostic Services
Once validated, the Patient Relations Officer will release the LOE and endorse the patient to the nearest or preferred diagnostic partner:
● Healthway Hospital / Healthway Clinic, or
● Hi-Precision Diagnostics
For areas without Healthway or Hi-Precision, the FREE 2D Echo may be done at the nearest hospital or clinic, with coordination through an IE Medica Product Specialist, in accordance with program guidelines.
